Birth Records

Obtaining Birth Records

Birth Records are not public information and can only be obtained by the Individual, a parent or a legal guardian.

In Person

Submit a Current Driver’s License or State ID, and fill out the application for a Birth Certificate in the County Clerk’s Office.

By Mail

Complete the Birth Record Request Form (PDF)or send a letter that includes the following:

  • Birth Name of person
  • Date of birth
  • Father’s name
  • Mother’s maiden name
  • A copy of your current Driver’s License or State ID.

Your letter or Birth Record Request Form should be submitted with payment (checks and money orders should be made payable to "Manistee County Clerk"), and a self addressed business sized stamped envelope to:

Record Costs

  • $15 for the first copy
  • $5 for each additional copy purchased at the same time/on the same day
